Good afternoon Leslie Frost Community,
Your child will soon be bringing home their Student Information Forms (SIF). The SIF is the document that lists key information for each student (e.g., contact numbers and email addresses). We ask that our families:
– carefully go over the information on this form (correcting and updating as required)
– please pay particular attention to parents/guardians contact numbers (cell phone and home phone numbers), emergency contact numbers (e.g., relatives or friends who can pick up your child if you are unavailable), and email addresses
– return this form to the school as soon as possible (your child will give it to their homeroom teacher, who will then bring it to the office)
It is absolutely critical that we have accurate contact information on file. We use the SIF to update our main school database and we can keep these forms on file at the school. This is our primary way to have the most up to date information as possible and we use forms as a paper backup if our electronic/online systems are not working. The SIF is sent home in paper format only (not electronically).
Below is also a PDF of the Permission Form For Personal Student Information. A paper copy of this form will be sent home in the paper package. This too needs to be updated each year now. If you have the appropriate software, the form can be filled out electronically; however, please still return to your child’s homeroom teacher.

Along with the SIF and permission form, a number of forms and information pieces will be sent home too over the next few days. Some of these forms do require updates and signatures; please fill out and return to your child’s homeroom staff. As always we appreciate your support in getting all of the appropriate paperwork filled out and returned!
